GitHub Actions health review

Find failed, disabled and unusually old workflow runs across a configured repository list without rerunning anything.

// what it does
  1. 01Confirm the scope. Require the repository list and review window. Record any owner-supplied expected cadence, branch or event filters. Do not infer that a manually triggered workflow should run on a schedule.
  2. 02Read workflows and runs. List the configured repositories' workflows, then read their recent runs within the limits. Preserve workflow IDs, state, run links, event, branch, timestamps and conclusions. Report permission and pagination gaps.
  3. 03Identify confirmed problems. List failed, timed-out or action-required runs and disabled workflows. Report canceled runs separately. Flag queued or in-progress runs older than the supplied threshold as needing investigation, not proof of a dead worker.
  4. 04Check expected activity. Only when an explicit cadence or expected run window is supplied, compare it with the observed runs for the same workflow, branch and event. Mark missing expected activity as unverified when the data window or permissions are incomplete. No runs is not itself a failure.
  5. 05Return the health report. Rank confirmed failures ahead of suspected stale activity, link every finding to the workflow or run, and name the missing evidence. Include workflows requested, assessed, with findings, and not fully checked. Recommend the next human investigation without claiming the root cause from a conclusion code alone.
// guardrails in the draft

Never enable or disable workflows, cancel or rerun jobs, edit workflow files, or change secrets.

Configure read-only grants; this report does not require Actions write access.

Do not download arbitrary artifacts, execute workflow code, or expose secrets from logs.

Do not claim complete coverage when a cap, permission failure or missing run history stopped the review.

An independent later run must re-read the current state before calling a previous finding resolved.
